Doug was John McCain’s stepson, the oldest child of his first wife, Carol McCain, from a previous relationship. After marrying Carole in 1965, John adopted Doug and his brother, Andrew. A year later, they had a daughter, Sidney, but shortly after, John’s plane was shot down during the Vietnam War, and he was held as a prisoner of war for almost six years, separating him from his family.

After being rescued and brought back to the United States, he became involved in politics, which his son Andy remembers sadly contributed to the end of his first marriage.

It broke my heart hearing him talk about it, but he explained that as his career took off, he was traveling constantly – all over the world. He admitted it really put a strain on things with his marriage, and honestly, it just made me ache for them. It was so sad to hear him be so vulnerable about it in that documentary, John McCain: For Whom the Bell Tolls. I’ve watched it a million times, and it still gets to me.

He met his future second wife, Cindy McCain, in Hawaii in 1979. She was 18 years younger than him.

Sidney acknowledged that age is irrelevant when it comes to love, saying, “You can’t help who you fall in love with.” In an HBO documentary, he explained that he genuinely believes both his father and Cindy are deeply in love, and he finds that touching. However, he admitted the situation was incredibly difficult at the time.

John and Cindy McCain expanded their family after their marriage, welcoming four more children: Meghan, Jack, James, and Bridget.

Doug opened up about his relationship with his younger siblings in the HBO doc.

He remembered it being an adjustment at first. He has good relationships with Cindy, as do his siblings. He also gets along well with Meghan, Jack, Jimmy, and Bridget, though they aren’t extremely close. However, he feels comfortable reaching out to them if he ever needs anything.
